Information for Coaches

• Emergency Coaching Permit Application
• Concussion Education Program (required for emergency and 5-year permit) – Mod 15, Mod 51 (football coaches only) and (refresher course MUST be taken during the 5th yr of your coaches application)
• Volunteer Coaching Application (required by the BOE)
• 45-Hour Blended Learning Coaching Course to Obtain a 5-Year Permit (all online courses must be completed before your only in person class)
• Renew a 5-Year Coaching Permit
• Coaching Modules (CEUs) to Maintain Coaching Certificate(Need to take 5 modules every years, cannot repeat mods)
• FINAL Forms Coaches Registration (MUST be filled out once a year prior to working with athletes)
• Print Out a CEU Certificate
• CIAC Home Page (enter tournament rosters, scores, penalties, pitches, etc.)
• Meriden Public Schools Volunteer Application
• Mod 15 (Concussion module, needed to take in order to get coaches cert.)
• Sexual Harassment Prevention Training (print out cert after you complete course and give to AD)
• DCF Mandated Reporting (print out cert. after you complete course and give to AD)
